If you’ve been off sick for more than 7 days

To request a sick note:

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

RUH  Fit Note

After you have left the Royal United Hospital (RUH) or been seen in outpatients you may need further support.

For example, the hospital teams at the RUH (rather than your GP) should provide you with a FIT note, where needed.

If you’re unwell and cannot work, you may need a fit note (sometimes called a sick note) for your employer.

If you did not receive a Fit note after your hospital visit or for other queries we encourage you to contact the department whose care you are or were under.
